He basically wants to be a SSG just for the medical and delay collecting his retirement for 15 years because he thinks it would be cheaper. He wants to be done with the army, but the value of the medical in his situation means that he wouldnt be able to afford medical insurance while running his family bussiness. He is now trying to resign his commission and come back as a SSG. The guy made the calculation that he would never be selected for COL, came off active duty as a LTC with just over 20 years into the reserves. An active duty LTC with a family and three EFMP kids that have a multitude of serious medical problems. One reserve retirement strategy I just ran into this week blew my mind.

Your active duty time can be calculated at 365pt per yea, and a good guesstimate is 100 points per reserve year, thought I would also look at a low of 50 just for comparrison. HRC has a reserve retirement calculator that you can use to make estimates.

As an AGR with 19.5 years of active duty and 3 years of TPU time, I think I have 7900ish points which would grant me about the equivalent of just under 21 years of active duty retirement.

The army reserve will put out an annual point sheet for you which tells you how many points you have. The value of each point changes with your time in service and grade at retirement (a SGTs points could be worth $.10 each vs a COL at $.45 each/not actual numbers, those also change regularly). The way reserve retirement works, you get 1 point for every day that you work in the army (2 points for each day during drill weekend).
